NIFTPÀÇ °³³ä

°©»ó¼± À¯µÎ¾Ï(papillary thyroid carcinoma, PTC)Àº 10°³ ³»¿ÜÀÇ º¯Á¾ ȤÀº ¾ÆÁ¾À» °¡Áö°í ÀÖÀ¸¸ç °¡Àå ÈçÇÑ º¯Á¾Àº ¿©Æ÷¼ºº¯Á¾(follicular variant papillary thyroid carcinoma, FVPTC)À¸·Î Àüü °©»ó¼±À¯µÎ¾ÏÀÇ ¾à 10-20%¸¦ Â÷ÁöÇÑ´Ù. (1) FVPTC´Â ´Ù½Ã ÇǸ·À» °¡Áö°í ÀÖ´Â FVPTC(Encapsulated FVPTC, EFVPTC)¿Í ÇǸ·À» °¡Áö°í ÀÖÁö ¾ÊÀº °Í Infiltrative FVPTC·Î ºÐ·ù°¡´ÉÇϸç EFVPTCµµ ÇǸ·Ä§À±, Ç÷°ü ħÀ± µîÀÇ ¼Ò°ßÀ» º¸À̴ ħÀ±¼º(invasive EFVPTC)°ú ºñħÀ±¼º(non invasive EFVPTC)À¸·Î ´Ù½Ã ºÐ·ùÇÒ ¼ö ÀÖ´Ù. ÇǸ·ÀÌ ÀÖ´Â °æ¿ì ¿µ»óÇÐÀûÀ¸·Î´Â isoechoicÇÑ ±ÕÁú¼ºÀÇ Á¾¾çÀ¸·Î ÁÖÀ§¿¡ hollow¸¦ º¸ÀÌ´Â ¿©Æ÷¼ºÁ¾¾çÀÇ ¼Ò°ßÀ» º¸ÀÌ´Â °æ¿ì°¡ ¸¹´Ù. º´¸®ÇÐÀûÀ¸·Î FVPTC´Â ÀϹÝÀûÀÎ PTCÀÇ ÇÙÀÇ ¸ð¾çÀ» °¡Áö°í ÀÖÀ¸¸é¼­ micro-follicular formationÀ» ÇÏ°í ÀÖ´Â, Áï ¿©Æ÷¼ºÁ¾¾ç°ú À¯»çÇÑ cyto-pathologic architecture¸¦ °¡Áö°í ÀÖ´Â Á¾¾çÀ¸·Î Á¤ÀÇÇÒ ¼ö Àִµ¥ infiltrative PTCÀÇ °æ¿ì ÀϹÝÀûÀÎ PTC¿Í µ¿ÀÏÇÑ ¿¹Èĸ¦ º¸ÀÌ´Â ¹Ý¸é non onvasive EFVPTCÀÇ °æ¿ì ±²ÀåÈ÷ ÁÁÀº ¿¹Èĸ¦ º¸Àδٴ »ç½ÇÀº Àß ¾Ë·ÁÁ® ÀÖ´Ù. (2) ÃÖ±Ù ¹Ì±¹ ÇÇÃ÷¹ö±× ´ëÇÐÀÇ Yuri Nikiforov ±³¼ö ÆÀ¿¡¼­ non invasive EFVPTCÀÇ °æ¿ì 109¸íÀÇ È¯ÀÚ Áß ¼ö¼ú ÈÄ ÃßÀû±â°£(10-26³â, median 13³â) Áß Àç¹ßÀ̳ª »ç¸ÁÀÌ ÇÑ °Çµµ ¾ø¾ú´Ù°í º¸°íÇϸ鼭 non invasive EFVPTC¸¦ Non Invasive Follicular Thyroid neoplasm with Papillary-like nuclear feature (NIFTP)·Î °³¸íÇÏ°í ¾Ï ºÐ·ù¿¡¼­ Á¦¿ÜÇÏ°í ¾ç¼ºÁ¾¾çÀ¸·Î ºÐ·ùÇÏÀÚ´Â ÁÖÀåÀ» ÇÏ¿´°í (3) Çѱ¹ÀÇ ÀϺΠ¾ð·Ð¿¡¼­ ¾Ï °úÀ×Áø´ÜÀÇ ÇÑ ¿¹·Î À߸ø ÀοëÇϸ鼭 »õ·Ó°Ô °ü½ÉÀ» ²ø°Ô µÇ¾ú´Ù.

NIFTPÀÇ ÀÓ»óÀû¿ë°¡´É¼º

NIFTPÀÇ °³³ä ÀÚü´Â ¹Þ¾Æµé¿©Áú ¸¸ ÇÑ °¡´É¼ºÀÌ ³ôÀ¸¸ç ÀÌ´Â ÀüÀûÀ¸·Î NIFTPÀÇ »ý¹°ÇÐÀûÀÎ ¼ºÁú°ú ÀÌ¹Ì Àß ¾Ë·ÁÁø ¾çÈ£ÇÑ ¿¹ÈÄ¿¡ ±âÀÎÇÑ´Ù. ±×·¯¹Ç·Î ¼ö¼ú ÈÄ Á¶Á÷°Ë»ç¿¡¼­ »õ·Î¿î º´¸® Áø´Ü ºÐ·ù·Î ¹Þ¾Æµé¿©Áú °¡´É¼ºÀÌ ³ô´Ù. ÇÏÁö¸¸ Á¤½Ä Áø´Üü°è¿¡ ÆíÀԵDZ⿡´Â ¼¼°èº¸°Ç±â±¸(WHO)ÀÇ ºÐ·ùü°è¿¡ ÆíÀÔÀÌ µÇ¾î¾ß ÇÏ°í ±¹³» Áø´Ü¸í ü°è KCD¿¡µµ ÆíÀԵǾî¾ß ÇϹǷΠ½Ã°£ÀÌ ÇÊ¿äÇÏ´Ù. ´Ù¸¸ ÀϺο¡¼­ À̾߱â ÇÏ´Â °Íó·³ ºÒÇÊ¿äÇÑ ¼ö¼úÀÌ ÁÙ¾îµé °ÍÀ̶ó´Â Á¡¿¡´Â µ¿ÀÇÇϱ⠾î·Æ´Ù. NIFTP´Â ÇöÀçÀÇ ±â¼ú·Î´Â ¼ö¼ú ÈÄ º´¸® °Ë»ç¿¡¼­ Á¾¾ç Àüü¸¦ Á¤¹Ð°Ë»çÇÏ°í ¶§·Î´Â ¸é¿ªÁ¶Á÷È­ÇбîÁö µ¿¹ÝÇØ¾ß Áø´ÜÇÒ ¼ö ÀÖ´Â °³³äÀ¸·Î Áø´ÜÇϱâ À§Çؼ­´Â ¼ö¼ú¿¡ ÀÇÇÑ Á¾¾ç ÀûÃâÀÌ ÀüÁ¦ µÇ¾î ÀÖ´Â °³³äÀÌ´Ù. 1) ¼ö¼ú Àü ÃÊÀ½ÆÄ ¼Ò°ßÀÌ NIFTP ȤÀº ¿©Æ÷¼º Á¾¾çÀ» ÀǽÉÇÒ ¼ö ÀÖ´Â ¼Ò°ßÀÏ °æ¿ì Å©±â°¡ Å©°í ¼ö¼ú Àü Áø´ÜÀÌ ÀϹÝÀûÀÎ °©»ó¼±À¯µÎ¾ÏÀ¸·Î Áø´ÜµÇ¾î °©»ó¼±ÀüÀýÁ¦¼úÀÇ ÀûÀÀÁõÀÌ µÇ´õ¶óµµ ȯÀÚ¿¡°Ô NIFTPÀÇ °¡´É¼ºÀ» ¼³¸íÇÏ°í ½ÅÁßÈ÷ °áÁ¤Çؼ­ ¾ç¼ºÁ¾¾ç¿¡¼­ °©»ó¼±À» ÀüÀýÁ¦ÇÑ °æ¿ì¿¡ »ý±æ ¼ö ÀÖ´Â ¹ýÀû, À±¸®Àû ¹®Á¦¿¡ ´ëºñÇÏ¿©¾ß ÇÒ °ÍÀÌ´Ù.

ÀÌ¿Ü¿¡ ºñÀÇÇÐÀûÀÎ ¹®Á¦·Î´Â ¼ö¼ú Àü °©»ó¼± À¯µÎ¾ÏÀ¸·Î Áø´ÜÇÏ°í ¼ö¼úÇÑ ÈÄ ÃÖÁ¾Áø´ÜÀÌ NIFTP·Î ³ª¿Â °æ¿ì ±¹³» »çº¸Çè¿¡¼­ º¸ÀåÇÏ°í ÀÖ´Â ¾Ïº¸ÇèÀÇ Àû¿ë ¹®Á¦°¡ ¹ý·üÀûÀÎ ¹®Á¦·Î ºñÈ­ÇÒ ¼ö ÀÖ´Â °¡´É¼ºµµ ÀÖ´Ù.
